The Pre-Participation Physical Evaluation (PPE) packet needs to be returned to the school office by Wednesday, May11th. Please completely fill out the front page of the PPE form, and the top of the second page. Also make sure you read, check appropriate boxes and sign the KSHSAA Eligibility Checklist and the KSHSAA Concussion & Head Injury Release Form (student and parent/guardian signatures must be on all three forms.) The sports physicals will be Wednesday, May 11th with Dr. Allen and Nicole Huey APRN.
This is your opportunity to get your sports physical completed free of charge. You may choose to get the sports physical completed at another time, but most insurance companies do not cover physicals.
